Cancellation Fees

Your appointment time is reserved exclusively for you. A late cancellation or missed visit not only affects our schedule but also creates an opportunity that could have been offered to another patient. To ensure we can accommodate everyone effectively, we kindly ask for at least 48 hours’ notice for any cancellations or changes to your appointment.

Please note the following:

Cancellations with Less Than 48 Hours' Notice: If you provide less than 48 hours’ notice or fail to attend your appointment, a cancellation fee of $125 will be applied, or a deposit will be required for future bookings.

Documentation Fees

We understand that there are times when you may require documentation for work or insurance purposes.

These fees help us cover the administrative costs involved in preparing and validating your forms.

Standard Work Note: A fee of $20.00 will be charged for a basic work note. - Employment
Documents: Fees range from $50.00 - $100.00 depending on the length and complexity of the document.
Insurance Forms: A fee of $150.00 will be charged for completing insurance forms.
Legal Documents: A fee of $150.00 will be charged for any legal documents requested.
